Berwyn Public Library

1. Schools must apply on an individual basis for a School Library Number for their building and accept responsibility for that number.

2. School loans are available on all library materials.

3. Teachers and other school employees must bring a School Loan Form each and every time materials are being checked out. No exceptions will be made. School Loan Forms must be completely filled out and signed by the school's designated authorized staff. Copied School Loan Forms will not be accepted. Each school will be provided with School Loan Forms on Library letterhead at the start of each school year. If your school depletes its supply of School Loan Forms, please call the Youth Services Department at 795-8000 extension 3006 for more.

4. The School Loan Period for Berwyn Public Library print materials is four weeks . If this loan period is not long enough for teacher needs, teachers are advised to contact the Youth Services Department to discuss an extended loan period. There will be no renewals on any materials.

The School Loan Period for Berwyn Public Library audiovisual materials is governed by the loan period policy. Videos checked out a School Library Card will circulate for one week .

5. Interlibrary loan requests by teachers will be accepted with the following limitations. Interlibrary loans are limited to teachers teaching in Berwyn schools. The school must possess a valid School Library Number. Loan periods on interlibrary loan items are governed by the lending library. Berwyn Public Library has no control over these loan periods. The Berwyn Library cannot guarantee these materials for a specific date.

6. You must notify your school library or the Berwyn Public Library Youth Services Department regarding the assignment one week before you need the material. If you contact your school librarian, she/he will drop off or fax the teacher loan form to the Youth Services Department. If a request is made with less than one week's notice, the Berwyn Public Library Youth Services Department cannot guarantee any materials will be ready in time.

7. The materials loaned are for classroom use only . The school is financially responsible for all materials loaned to the teacher.

8. Berwyn Public Library is here to serve the general public. The Library reserves the right to limit the number of materials borrowed so not to deplete its collection in any given subject area.
